Dry Needling Consent Form

As with any medical procedure, Dry Needling comes with the potential risk of complications.

It is important that we as clinicians thoroughly educate our patients regarding, not only the potential benefits of Dry Needling, but also the potential complications.

While complications due to Dry Needling are uncommon, they do sometimes occur and must be considered prior to giving consent to the procedure.

Thus, following education, it is critical that we acquire both our patient’s verbal & written consent prior to treatment — indicating our patient has sufficiently bought in & approved of the selected treatment approach!
